The Kentucky Agricultural Experiment Station in the Martin-Gatton College of Agriculture, Food and Environment conducts programs funded in part by the U.S. Department of Agriculture (USDA). To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Reasonable Accommodation
The Americans with Disabilities Act of 1990 (ADA) is a federal anti-discrimination statute that provides civil rights protection to individuals with disabilities in employment, public accommodations, State and local government services, and telecommunications.
Employees
If you are a qualified University faculty member or staff employee with a disability, you may request a reasonable accommodation from your supervisor, chair, dean, or the Office of Campus Accessibility and ADA by completing a Request for a Reasonable Accommodation Form. When the disability is not obvious, you may be asked to submit sufficient medical documentation.
